Keep it simple: phaco alone
Lens extraction has a high probability of reducing IOP in angle closure disease, and can realistically be used as a standalone surgical intervention in selected cases.
In open angle disease, data drawn from studies lacking e.g. gonioscopy do not allow informed conclusions. IOP lowering after lens extraction in POAG is of only mild-moderate amount, is unpredictable and is often not sustained. As a specific surgical strategy to treat POAG, combination with a glaucoma operation is more often needed.
